Presentation

Teenage patient, upper right lateral incisor in crossbite behind the lower canine, mild lower crowding, Class I molars.

Planning approach

  1. Space created for the lateral with 0.8 mm of IPR and a small amount of expansion.
  2. Lateral proclined over the bite in the first 8 stages, with a bite ramp built into the aligner to disclude.
  3. Lower arch aligned in parallel to remove the interference.

Setup outcome

Crossbite corrected in the setup with the lateral in positive overjet and contacts restored.

Class II div 1, moderate upper crowding, patient refuses extractions. Is this doable with aligners?

  2. Yes. I would stage sequential distalisation and start Class II elastics from stage 1. Send the lower scan and photos and I will build a setup you can review.

  3. Sending now. Can we keep the lower incisors where they are?

    14:21
  4. We can limit proclination to about 2° and hold the rest with the elastics. I will show you both versions.
