Presentation

Young adult with a 2.5 mm midline diastema and generalised upper spacing. Lower arch aligned. Overjet 3 mm, acceptable overbite.

Planning approach

  1. Spaces closed with reciprocal movement, retracting the anterior segment while holding the molars.
  2. Attachments placed on the lateral incisors and canines for bodily movement rather than tipping.
  3. Frenectomy referral noted in the plan so the diastema does not reopen.

Setup outcome

Spaces closed in the setup with the midline centred and overjet maintained at 2.5 mm.

Class II div 1, moderate upper crowding, patient refuses extractions. Is this doable with aligners?

  2. Yes. I would stage sequential distalisation and start Class II elastics from stage 1. Send the lower scan and photos and I will build a setup you can review.

  3. Sending now. Can we keep the lower incisors where they are?

    14:21
  4. We can limit proclination to about 2° and hold the rest with the elastics. I will show you both versions.
