Presentation

Adult with a 3 mm anterior open bite from canine to canine, tongue thrust habit, Class I molars. No skeletal surgery wanted.

Planning approach

  1. Relative intrusion of the posterior segments using the aligner as a posterior bite block.
  2. Anterior extrusion staged slowly with horizontal attachments on the incisors.
  3. Habit management and myofunctional referral noted as a condition of the plan.

Setup outcome

Positive overbite of 1.5 mm in the setup with posterior contacts preserved.
